First correction: 2026 NCLEX-RN is 85-150 items, not 75-145

The April 2026 NCLEX Candidate Bulletin states that the NCLEX-RN is a variable-length computerized adaptive test with 85 to 150 items and a five-hour time limit, including optional breaks.

The 2026 RN Test Plan is effective from April 1, 2026 through March 31, 2029. Candidates should treat the test plan as the map for study, because it explains client-needs categories, clinical judgment, sample item formats and item-writing logic.

For Ahpra/NMBA Stream B registered nurse candidates, the RN MCQ examination is coordinated with NCSBN and delivered through Pearson VUE using the NCLEX-RN.

What Taiwan-trained nurses should train differently

Many Taiwan-trained nurses are clinically experienced but need to adjust to NCLEX language: prioritisation, delegation, safety, therapeutic communication, infection control, pharmacology and US-style entry-level nursing responsibility.

The exam rewards clinical judgment more than memorisation. A useful daily question is not simply 'what is correct?' but 'what is the safest nursing action now, based on the cues available?'

Build a bilingual weak-point log for English medical terms, medication classes, maternal-child nursing, mental health, paediatrics, delegation and emergency prioritisation.

NGN item formats and scoring

Next Generation NCLEX item formats may include case studies, bow-tie, matrix/grid, drag-and-drop, highlight and extended multiple response. The point is to assess whether the candidate can notice cues, analyse them, prioritise hypotheses, take action and evaluate outcomes.

Do not rely on the old rule that all SATA items are simply all-or-nothing. NCSBN states that items with more than one key can use partial-credit scoring, including plus/minus, zero/one and rationale scoring models.

For practice, this means reviewing rationales is more valuable than counting only the percentage correct. A candidate should know whether the mistake was knowledge, cue recognition, prioritisation, English wording or unsafe delegation.

A realistic 8-12 week plan

Weeks 1-3 should focus on the official test plan, baseline question bank results and high-frequency content gaps. Use daily blocks of 50-75 questions plus rationale review rather than passive reading.

Weeks 4-8 should increase to timed mixed sets and CAT-style practice. Track each missed item under content area and reasoning error. Pharmacology, physiology, maternal-child, mental health and safety/delegation usually need repeated spaced review.

The final two to four weeks should focus on full timed practice, NGN case studies, sleep rhythm, test-day routine and reducing avoidable anxiety. Readiness scores from commercial tools are signals, not guarantees.

Stream B timeline and OSCE connection

Ahpra/NMBA advises that once a portfolio has been approved, the IQNM dashboard will direct candidates to register and pay for the NCLEX-RN through Pearson VUE. After authorisation, candidates schedule the examination through Pearson VUE.

The current Ahpra RN candidate handbook states that the Authorization to Test is valid for 90 days. NCSBN also reminds candidates that they must test within the dates of the ATT, which cannot be extended.

Passing the NCLEX-RN is not the end of the Stream B route. Stream B candidates still need to complete the OSCE before they are eligible to apply for registration in Australia.
